HOME HEALTH
Home health care services are prescribed by a physician to help with rehabilitation after an illness, injury, hospital stay, or surgery, or to help manage acute or chronic medical conditions with the goal of preventing an unnecessary hospitalization. Home health care visits are generally covered by Medicare/Medicaid through the Medicare home health benefit or by private insurance.
A Thrive expert will discuss what is needed and covered by Medicare or private insurance, and then work with your doctor to develop your care plan which may include:
- Nurses for medical needs
- Physical Therapists (PT) to help restore mobility
- Occupational Therapists (OT) to improve the ability to perform every day activities
- Speech-Language Pathologists (SLP) to help with speech, language, voice, cognition, and swallowing issues
- Medical Social Workers (MSW) to help a client and their family connect with community resources
- Home Health Aides (HHA) to provide bathing assistance & related personal care on an intermittent basis
